Nephrology & Special Care


Nephrology specialty care is by referral only from your primary care physician. For your first appointment we need the primary care referral, reason for being referred, your most recent labs and any radiology done pertaining to your kidneys. Our staff is happy to assist you with setting us your Nephrology appointment and can be reached at 707-252-8407.

PRIMARY CARE:

The healthcare system is rapidly changing as advances are made in medical science which helps us to better meet our patient needs. One such change is the transition away from the “head-to-toe” annual physical. Wellness exams now focus on preventative care that research has shown to be effective in improving the health of our patients.  

ALL PATIENTS ARE ENCOURAGED TO HAVE THEIR ANNUAL WELLNESS EXAM.


At your wellness exam you can expect to:
  • Update medical and family history
  • Have your weight, height, and blood pressure measured
  • Review your list of health conditions
  • Review the medications you take.
  • Screening for cognitive impairment
  • Discuss what health screenings you may need based on your age and medical history
  • Discuss immunizations that you may need to prevent future illness.
At the end of your visit you will receive a printed care plan summarizing the visit, your health conditions and your regular medications.

A wellness exam does not deal with new or existing health problems these require an additional visit to our office. Most insurances pays for an annual wellness exam to discuss preventative care and health promotion, but if other health concerns are addressed at the visit an additional office visit charge may be billed to your insurance and you could owe a copayment or deductible.
